The authors examined sibling discussion about safe sexual practices and the extent to which such discussions influence adolescents' attitudes toward safe sex, their self-efficacy for refusing sex, communicating about condom use with partners, and buying and using condoms. Participants were 297 Midwestern high school students, approximately 17 years old, who answered questionnaires and who had a biological sibling at least one year older but younger than 30. Of the 297 students, 62 percent were female. Sixty-eight percent reported having engaged in sex at least once, while 32 percent reported being virgins. Eighty-one percent were Caucasian, 16 percent African-American, and 3 percent of another ethnicity. Older siblings were, on average, 21.97 years old. "We found that when sibling discussions about safe sex practices take place in concert with parent-adolescent conversations abut sex, adolescents reported less risky attitudes about appropriate sexual behavior for people their age and higher levels of selfefficacy for communicating with partners about condom use and for buying and using condoms, " the researchers wrote. "We did not find similar results with regard to adolescents' perceived ability to refuse sex." The authors said their results reinforce previous research emphasizing that parentchild communication about sexuality is important, and also highlighted that siblings' discussions could play "a supplemental role in influencing adolescents' sexuality." The researchers suggested that while sibling conversations about sex do not substitute for parent-child communication about sex, they may be an important supplement. They noted that older siblings might be able to facilitate parent-child discussions regarding sexual issues, or teach younger siblings the importance of such discussions. "Older siblings are in the unique position of being able to explain to parents the sexual situations that their younger siblings are likely to encounter and the potential barriers to safe sex practices that they may experience, " the authors noted. The researchers suggested that older siblings might be integrated into existing programs that emphasize that parent-child communication is associated with more positive outcomes for adolescents' responsible sexual behavior, "so that they too learn about their important role in the development of their younger siblings' attitudes about and selfefficacy for safe sex practices.

10% of patients with symptomatic IPF, the chest radiograph may be entirely normal. Radiographic findings in patients with IPF are limited to the lung fields. Hilar lymphadenopathy or pleural effusion suggests a different cause of diffuse infiltrative lung disease. HRCT may be helpful at this stage of the evaluation because the pattern of abnormalities seen in IPF is specific, and many of these other causes have characteristic and very different patterns.6 Also, HRCT is likely to reveal abnormalities in cases in which the chest radiograph is normal. The HRCT pattern in IPF includes patchy, peripheral, subpleural, bibasal reticular abnormalities with minimal groundglass opacity. With advanced disease, honeycomb lung and traction bronchiectasis bronchiolectasis, indicating end-stage lung fibrosis, are seen. Extensive areas of ground-glass haziness, indicating an acinar filling process, are not characteristic of IPF but may be seen in AIP, LIP, DIP RBILD, and NSIP F. Pulmonary function testing The physiologic abnormalities of IPF are basically the same as those described for diffuse infiltrative lung disease. The classic composite physiologic picture of IPF is reduced DLCO, restriction of lung volume, exercise-induced oxygen desaturation, and absence of airflow obstruction. Some of the alterations correlate with the pathologic findings.50 Histopathologic features The histopathologic features of IPF are nonspecific. Similar histopathologic findings are observed in diffuse infiltrative lung disease associated with collagen vascular disease, various types of drug-induced diffuse disease, chronic hypersensitivity pneumonitis, asbestosis, and other disorders. Chronic fibrosis and a scant inflammatory cell infiltrate, the essential pathologic features of IPF, are common reactions to a variety of agents. The histopathologic picture in IPF is termed usual interstitial pneumonitis UIP ; and is characterized by minimal interstitial inflammatory round cell infiltrate, widening of alveolar septa, and fibrosis with fibroblastic foci43 [see Figure 8]. The distribution of the lesion is irregular: areas of intense fibrosis can coexist with areas of near-normal lung in the same open lung biopsy specimen. In some cases, more than one histologic pattern is seen; in such cases, if any areas of UIP are seen, the clinical diagnosis should be IPF.51 UIP must be differentiated from DIP RBILD, AIP, LIP, NSIP F, and BOOP, because of different prognoses 43 and treatment approaches see below ; . Lung biopsy Transbronchial lung biopsy is a sensitive tool for diagnosing sarcoidosis and lymphangitic cancer and may reveal pathologic features suggestive of IPF i.e., fibrosis and widened alveolar septa with scant inflammatory cell infiltrate ; . Open lung biopsy is needed to exclude other causes of diffuse disease. It is up the clinician to decide whether to proceed to open lung biopsy when clinical evaluation and transbronchial lung biopsy are highly suggestive of IPF. Open lung biopsy is advisable when there is any doubt about whether there is an infectious cause. Open lung biopsy should usually be performed in younger patients to establish the diagnosis of the underlying disorder with a reasonable degree of certainty.5 Older patients with typical clinical features and a compatible transbronchial lung biopsy result may reasonably be spared the morbidity associated with open lung biopsy. In recent years, techniques have been developed for measuring bulk rheologic viscoelastic ; , surface or interfacial ; , and transport properties of mucus or sputum. There is evidence that both the rheologic and surface properties of mucus can affect mucociliary clearance; with increasing epithelial damage and inflammation, these properties become progressively abnormal.2 4 Mucus is a viscoelastic gel consisting primarily of water and high-molecular-weight, crosslinked glycoproteins that form a tangled network. Respiratory mucus is usually cleared by cilia. Sputum, which is mucus mixed with inflammatory cells, cellular debris, and often bacteria, is generally cleared by coughing. The rheology of mucus is its capacity to undergo flow and deformation.4 A true solid responds to a stress with a finite elastic deformation that is totally recovered after the stress is removed. A liquid responds to a stress with viscous deformation, flowing continuously for the time that the stress is applied. After the removal of the stress, the flow ceases and there is no recovery of the strain. A viscoelastic gel such as mucus initially stores energy; with continued stress, it will begin to flow like a liquid. The viscoelastic behavior of mucus was thought to be one of the major determinants of mucociliary clearance. Studies using mucus simulant gels made from crosslinked vegetable gums ; suggest that MCTR on the frog palate is impeded by increasing mucus G * , but increases with greater mucus recoil.21 However, the relationship between the properties of actual airway secretions and MCTR is unclear. Giordano and colleagues22 studied the tracheal mucus velocity of dogs and found a negative correlation between the tracheal clearance rate and.

We previously reported that overexpression of PKC may predict TAM treatment failure [40] and therefore may be a prognostic indicator for TAM-resistance. Furthermore, the E2-induced tumor regression observed in our pre-clinical xenograft model suggests that perhaps selection of patients with PKC-overexpressing tumors may greatly improve the efficacy of high dose estrogen treatment, a therapeutic approach likely to be superior to TAM in this situation. Perhaps treatment of breast cancer with the PKC antisense oligonucleotide Affinitak LY900003 ISIS 3521 ; [59] can be improved by identification of patients that exhibit PKC overexpression. Finally, the recent favor of aromatase inhibitor therapy over TAM [60-62] is another potential concern for patients harboring tumors that overexpress PKC.
